Accommodation Request Form

The Accommodation Request Form is designed to be provided to students with disabilities who require accommodations to participate in an extra-curricular activity. The form allows the student to check off accommodations that are frequently requested and provide details on any additional needs they might have.

This document should be submitted to the event planner so that they may make the appropriate arrangements prior to the event.
