Description	Independent Living Montreal will be offering this workshop on the Management of Chronic Health Conditions. Information and registration session for the My Tool Box Program (www.facebook.com/mytoolbox.latelier), with Isabelle Ducharme. � Learn about the MUHC�s My Tool Box program, which we plan to bring to IL-M this winter. � Learn tips and tricks to help manage chronic conditions. � Share their experiences and ask questions on the topic of managing chronic conditions. A chronic condition is a long-lasting condition that can be controlled but not cured. Most common conditions include: asthma, diabetes, epilepsy, hypertension, hypothyroidism, multiple sclerosis, Parkinson's, rheumatoid arthritis, schizophrenia, lupus, and bipolar mood disorder.
